Clinical Notes Templates for SOE Exact (UK): Paste-Ready Workflow
Nosht
Published by Nosht Limited
SOE Exact is one of the most established on-premise PMS platforms in UK general practice. Its notes field is a free-text box — this guide covers how a structured notes app fits into the Exact workflow, exactly where the note lands, and the honest limits of that integration.
Why pair a notes app with SOE Exact
SOE Exact (Software of Excellence) handles appointment scheduling, charting, billing and recall for UK practices, many of them long-established. Like most PMS notes fields, Exact's clinical notes box is a blank text area — it captures whatever you type, with no prompts for the elements GDC Standards and FGDP(UK)/CGDent guidance expect documented.
A structured notes app produces the complete, compliance-prompted note first; you then paste the finished text into Exact's notes field. Exact keeps doing what it does well — the patient record, charting and billing — while the notes app handles documentation structure.
The copy-paste workflow, step by step
SOE Exact is on-premise software with no public API for third-party clinical-notes apps, so copy-paste is the only realistic integration — and it works reliably because every PMS notes field accepts pasted plain text.
- In your notes app, pick the template matching the treatment (e.g. composite filling, exam, extraction)
- Select your findings from the structured options
- Click "Copy" to copy the complete note to your clipboard
- Switch to SOE Exact — open the patient's clinical notes screen
- Click into the notes field and paste (Ctrl+V)
- Save the note in Exact as normal
Where the note lands, and what to check
- The pasted text lands in Exact's own clinical notes field — it becomes part of Exact's patient record exactly as if you'd typed it, with no separate storage.
- Exact's patient demographics, charting and appointment history stay exactly where they are — a structured notes app should only ever produce the note text, never duplicate patient details.
- Check the paste renders as plain text in Exact — a notes app that outputs rich formatting or markdown can paste awkwardly into some on-premise notes fields; plain text pastes cleanly everywhere.
- As an on-premise system, SOE Exact typically runs on practice-network machines — run your notes app in the same browser session at the same workstation for the fastest handover.
Honest limits
Be clear-eyed about what this integration is and isn't: it is copy-paste, nothing more. There is no API connection between a third-party notes app and SOE Exact, and no partnership or official integration agreement. Any claim of "direct SOE Exact integration" from a notes-app vendor should be demonstrated live in Exact before you believe it — Software of Excellence does not publish a general third-party API for notes apps to connect to.
The trade-off is reliability: copy-paste cannot break with a platform update the way an API integration can. It costs a few seconds per note and asks nothing of your SOE Exact administrator.
Which templates to use
Use whichever template matches the treatment you're documenting — examination, restorative (filling, crown), endodontics, oral surgery (extraction), periodontal, and the rest. Nosht ships 50 templates covering general practice, each producing plain text that pastes cleanly into Exact's notes field.
Where Nosht fits with SOE Exact
Nosht is built for exactly this copy-paste workflow: select your findings, copy the finished note, paste into SOE Exact's notes field, save. Clean plain text, every time.
Patient demographics and the record itself stay in SOE Exact, where they belong. Nosht's clinical notes hold no patient identifiers — only clinical descriptors — and no audio is captured. There are two exceptions, both optional: the Referral Letter template, where you choose to enter patient details (name and date of birth, with NHS number, address and contact optional); and Send Leaflet, which processes a patient's email address or mobile number solely to deliver a patient information leaflet.

Frequently asked questions
Does SOE Exact have a built-in structured notes feature?
SOE Exact's clinical notes field is a free-text box, with no structured, prompted templates built in. Practices either type notes directly (structure varies clinician to clinician) or pair Exact with a dedicated notes app and paste the finished note in.
Does Nosht integrate directly with SOE Exact via an API?
No. SOE Exact does not publish a general third-party API for clinical-notes apps, and Nosht has no partnership or direct integration with it. The method is copy-paste — reliable, and it works the same way with any PMS.
Is copy-pasting from a notes app into SOE Exact secure?
Your clipboard is local to your device — nothing is transmitted during the copy-paste step itself. Overall security depends on where the notes app processes and stores the note before you paste, and on SOE Exact's own security where the final note is saved. Choose a notes app that keeps patient identifiers out of the clinical note to minimise what it ever holds.
How long does the SOE Exact + notes app workflow take?
The copy-paste step itself takes a few seconds: open the template, select findings, copy, switch to Exact, paste, save. Most of the time is writing the note, which depends on the case — time it on your own appointments.
