Clinical Notes Templates for R4 (Carestream) UK: Paste-Ready Workflow
Nosht
Published by Nosht Limited
R4, from Carestream Dental, is widely used across UK general and specialist practice. Its notes field captures free text with no documentation prompts — this guide covers the copy-paste workflow, where the note lands, and the honest limits of integrating a notes app with R4.
Why pair a notes app with R4
R4 handles charting, treatment planning, imaging and billing for many UK practices, including larger and specialist ones. Like other PMS platforms, its clinical notes field is free text with no built-in prompts for GDC Standards or FGDP(UK)/CGDent record-keeping elements.
A structured notes app fills that gap: produce the complete note with its own compliance prompts, then paste the finished text into R4's notes field. R4 continues to hold the patient record, charting and imaging exactly as before.
The copy-paste workflow, step by step
R4 is an on-premise system with limited integrations available to third-party notes apps, so copy-paste is the realistic method — reliable because any PMS notes field accepts pasted plain text.
- In your notes app, select the template for the treatment (exam, filling, extraction, and so on)
- Select your findings from the structured options
- Click "Copy" to copy the complete note to your clipboard
- Switch to R4 and open the patient's clinical notes screen
- Click into the notes field and paste (Ctrl+V)
- Save the note in R4 as normal
Where the note lands, and what to check
- The pasted text lands in R4's own clinical notes field, becoming part of R4's patient record exactly as if typed directly.
- R4's charting, imaging and treatment-planning data stay exactly where they are — the notes app should only ever produce the note text.
- Check the paste renders as plain text — avoid notes apps that output markdown or rich formatting if you see it pasting awkwardly into R4's notes field.
- R4 typically runs on practice-network workstations — run your notes app in the same browser session at the same machine for the fastest handover.
Honest limits
This is copy-paste, nothing more — there is no API connection between a third-party notes app and R4, and no partnership or official integration agreement with Carestream. Treat any vendor claim of "R4 integration" with scepticism unless it is demonstrated live in your own R4 system.
The advantage of copy-paste is that it cannot break when R4 is updated the way a tighter API integration could — it costs a few seconds per note and needs no sign-off from your R4 administrator or Carestream.
Which templates to use
Use the template matching the treatment you're documenting — examination, restorative, endodontics, oral surgery, periodontal, prosthodontics and the rest. Nosht ships 50 templates covering general and much of specialist practice, each producing plain text that pastes cleanly into R4's notes field.
Where Nosht fits with R4
Nosht is built for this copy-paste workflow: select your findings, copy the finished note, paste into R4's notes field, save. Clean plain text every time, with no formatting to clean up.
Patient demographics, charting and imaging stay in R4, where they belong. Nosht's clinical notes hold no patient identifiers — only clinical descriptors — and no audio is captured. There are two exceptions, both optional: the Referral Letter template, where you choose to enter patient details (name and date of birth, with NHS number, address and contact optional); and Send Leaflet, which processes a patient's email address or mobile number solely to deliver a patient information leaflet.

Frequently asked questions
Does R4 have a built-in structured notes feature?
R4's clinical notes field is a free-text box with no structured, prompted templates built in. Practices either type notes directly or pair R4 with a dedicated notes app and paste the finished note across.
Does Nosht integrate directly with R4 via an API?
No. Nosht has no partnership or direct integration with R4 (Carestream). The method is copy-paste — reliable, and the same method that works with any PMS.
Is copy-pasting from a notes app into R4 secure?
Your clipboard is local to your device — nothing is transmitted during the copy-paste step. Overall security depends on where the notes app processes and stores the note before you paste, and on R4's own security where the final note is saved.
How long does the R4 + notes app workflow take?
The copy-paste handover itself takes a few seconds; most of the time is spent writing the note in the template, which depends on the case — time it on your own appointments.
