Dental Notes Audit Tool: Check Notes Against GDC & CGDent Guidance

A dental notes audit tool checks a clinical note for the documentation elements that GDC Standards and FGDP(UK)/CGDent record-keeping guidance expect — consent, risks, doses, post-operative instructions and more — and flags what's missing before you copy the note into your PMS. Nosht's version is called Bulletproof Mode, built into every template at no extra cost.

What Bulletproof Mode Checks

Bulletproof Mode runs a set of rules against the note you're writing, scoped to the template and treatment type. Each rule is informed by a named source — here are representative examples, not the complete list:

AreaExample checkInformed by
ConsentFlags a treatment note with no documented consent discussion, or missing risks/alternatives for procedures where GDC guidance expects them recorded.GDC Standards for the Dental Team; Montgomery v Lanarkshire Health Board
Local anaestheticFlags a treatment requiring LA with no agent, dose or volume recorded.SDCEP / BNF
Medical emergenciesFor an anaphylaxis entry, flags a missing adrenaline dose, route or time.Resuscitation Council UK
RadiographsFlags a radiograph taken with no clinical justification recorded.IR(ME)R 2017 / FGDP(UK) selection criteria
PeriodontalFlags a perio entry with no BPE score or staging/grading recorded where expected.BSP (2017 World Workshop) guidelines
Record-keeping basicsFlags a note missing patient ID, date, presenting complaint or post-operative instructions.FGDP(UK)/CGDent Clinical Examination and Record-Keeping

Every rule in Nosht's codebase carries its own source citation (e.g. "SDCEP Drug Prescribing for Dentistry", "BSP 2017", "Resuscitation Council UK 2021") — the examples above illustrate the range, not an exhaustive list of what's checked for every template.

A Sample Flagged Note

Say a clinician completes a composite filling note but doesn't record the local anaesthetic dose or the post-operative advice given. Bulletproof Mode surfaces both before the note is marked complete:

LA type/dose not recorded for a procedure requiring anaesthesia (SDCEP / BNF)

Post-operative instructions not documented (FGDP(UK)/CGDent)

What It Does Not Check

  • Whether the diagnosis or treatment plan was clinically the right call — that's a clinical judgement, not a documentation check.
  • Whether consent was validly obtained in substance — it checks whether the consent field was completed, not the quality of the conversation it summarises.
  • Anything outside the template's own structured fields — free-text additions aren't rule-checked.
  • Whole-practice compliance — CQC readiness, staff training records, equipment logs and policies are a different category of tool.

Bulletproof Mode is advisory, not a guarantee. It checks a fixed rule set informed by GDC and FGDP(UK)/CGDent guidance; it does not guarantee a note would satisfy a GDC case examiner, and it is not "GDC-approved" (the GDC does not endorse software). Clinicians remain solely responsible for their clinical decisions and the accuracy of what they document.

Frequently Asked Questions

What is a dental notes audit tool?

A dental notes audit tool checks a clinical note against a set of rules informed by GDC Standards and FGDP(UK)/CGDent record-keeping guidance, and tells you which expected elements — consent, risks discussed, doses, post-operative instructions and the rest — are present or missing before you copy the note into your PMS. It is advisory: it checks for the presence of documented elements, not the clinical appropriateness of the treatment itself.

Is Bulletproof Mode GDC-approved?

No audit tool can be "GDC-approved" — the GDC does not endorse software. Bulletproof Mode's rules are informed by GDC Standards for the Dental Team, FGDP(UK)/CGDent Clinical Examination and Record-Keeping guidance, and specialty body guidance (BSP, SDCEP, BNF and others) depending on the template. Each rule in Nosht cites its source.

Does Bulletproof Mode guarantee my notes are compliant?

No. It flags whether expected documentation elements are present using a fixed rule set — it cannot judge whether the clinical decision itself was appropriate, and it cannot catch everything a GDC case examiner might look for. Clinicians remain solely responsible for their clinical decisions and the accuracy of what they document. Treat Bulletproof Mode as a second pair of eyes, not a sign-off.

What does Bulletproof Mode NOT check?

It does not check whether the diagnosis or treatment plan was clinically correct, whether consent was validly obtained in substance (only whether a consent field was completed), free-text wording quality beyond the structured fields, or anything outside the template's own fields. It is a completeness and consistency check against a fixed rule set, not a clinical-judgement or legal-compliance guarantee.

Is a dental notes audit tool the same as practice compliance software?

No. Practice-compliance platforms (CQC readiness, policies, staff training records, equipment logs) cover the whole practice. A dental notes audit tool like Bulletproof Mode checks one clinical note at the point you write it, against record-keeping guidance — it is not a replacement for practice-wide compliance management.
